进展性缺血性脑卒中的发生率及其与不同病因分型的关系

摘要
目的观察急性缺血性脑卒中患者脑卒中进展的发生情况及不同TOAST分型进展性脑卒中发生率的差异。方法全面登记急性缺血性脑卒中患者一般特征、各种化验检查和影像检查结果。根据欧洲进展性卒中研究小组定义的进展性脑卒中诊断标准判定是否进展,依据相关临床资料进行TOAST病因分型。结果连续登记627例急性缺血性脑卒中患者,符合纳入标准的有569例(90.7%),127例为进展性脑卒中,发生率22.3%(127/569),其中大动脉型发生率35.6%(21/59)最高,小动脉型9.5%(21/222)最低,各型发生率之间比较有统计学差异(χ2=38.00,P<0.05)。结论急性缺血性脑卒中患者进展性脑卒中的发生率为22.3%,其中大动脉型发生率最高,各型发生率之间比较统计学差异,重视对进展性脑卒中的早期干预有利于改善患者预后。
