医保政策精准扶贫效果研究——基于URBMI试点评估入户调查数据

摘要：

本文基于精准扶贫的视角并以家庭为考察对象,采用处理效应模型对现有城镇居民基本医疗保险制度(城居保)的扶贫效果进行了综合评估,并对大病冲击下城居保对不同阶层的收入影响及其滞后效应进行了动态分析。研究发现,城居保政策对低收入城镇家庭具有明显的扶贫效果,尤其是对受到大病风险冲击的困难家庭,在避免出现因病致贫、因病返贫问题上能够起到显著的缓解作用,而且这种作用随着时间延续并通过影响家庭教育培训支出和劳动力健康状况还有进一步增强的趋势,但其扶贫效果在不同收入家庭具有明显的异质性特征,对中高收入参保家庭的影响尤甚,出现了"目标上移"现象,扶贫的精准性与预期存有差距,这为未来的精准扶贫工作指明了改进方向。
