一期机械性吸栓加支架植入术治疗髂股深静脉血栓形成的临床观察

摘要
目的评价一期机械性吸栓(PMT)加支架植入术治疗髂股深静脉血栓形成的效果。方法对24例髂股静脉血栓形成患者行PMT加支架植入术治疗,治疗前均行下肢静脉顺行造影或静脉彩超进行评估。结果溶栓后获完全溶解Ⅲ级20例(83.3%),Ⅱ级3例(12.5%)和Ⅰ级1例(4.2%)。溶栓时间平均约为0.25小时,尿激酶平均用量为16.7万U。6例植入下腔静脉滤器。溶栓后患、健侧大腿平均周径差为(1.38±1.61)cm,小腿为(0.80±0.87)cm,术后患肢肿胀症状明显减轻21例。12例术后出现肉眼血尿,无黑便、血便;2例有静脉穿刺点周围少量皮下淤血斑。无致命性肺梗死;无脑出血并发症、无穿刺部位渗血;支架植入术中无静脉破裂及支架移位等发生。结论 PMT加支架植入术治疗髂股静脉血栓形成具有治疗时间短、大出血风险低、溶栓药物用量低等优点,是一种具有良好前景的治疗方法。
