中期照护改善老年患者日常生活自理能力

摘要
目的探讨应用功能评估判定老年中期照护服务对象,分析中期照护对老年患者日常生活自理能力及肢体功能的影响。方法因急性病住院年龄大于60岁的患者,入院1周之内应用日常生活活动能力(ADL)量表(Barthel指数)及肢体功能测定进行评估。因本次急性病导致日常活动能力下降,肢体功能下降,Barthel指数45-95分,且具有康复潜能的患者纳入中期照护。实验组331例,于住院后3~7d内开始实施中期照护。对照组51例,实行常规急性病医疗护理。出院前重复ADL评估,出院后1个月再次行ADL量表及肢体功能测定予以随访评估。结果入院时与出院前比较,实验组ADL评分差异具有统计学意义(78.4±19.6 vs 83.8±16.2),对照组(83.0±13.2 vs 83.4±12.2)差异无统计学意义。入院时与出院后1个月比较,实验组握力有显著性变化,步速及伸展测试的改善无统计学差异。对照组3项指标的前后比较均无统计学意义。入院时与出院后1个月比较,实验组和对照组ADL评分的改善均具有显著意义。两组ADL评分增加量的比较(15.15±19.71 vs 10.14±11.27),两组间差异具有统计学意义。结论实施中期照护有利于改善老年住院患者日常生活自理能力,提高握力。ADL评分及肢体功能评估有利于判定因急性病住院的老年患者是否适合进入中期照护,同时有利于判定中期照护的实施效果。
