摘要：

目的探讨垂体前叶激素水平对急性脑梗死患者预后的影响。方法对发病72 h以内的183例急性脑梗死患者进行皮质醇、三碘甲状腺原氨酸(T3)、游离甲状腺素(fT 4)、促甲状腺素(TSH)和生长激素(GH)进行测定并评估其对预测3个月和6个月后神经功能的价值。结果与其他激素相比,预后价值准确性和NIHSS评分范围最好为皮质醇。皮质醇是神经功能预后独立标志物(OR=1.0,95%CI 1.00～1.01,P<0.000 2,校正年龄和NIHSS),但对NIHSS临床价值无协同意义。结论皮质醇是对急性脑梗死患者3个月和6个月神经功能预测的独立预后标志物。
