尿酸与急性缺血性脑卒中预后的前瞻性病例研究

摘要
目的探讨血清尿酸水平与急性缺血脑卒中患者临床预后的关系。方法以388例急性缺血性脑卒中患者为研究对象, 收集人口统计学、生活方式、疾病史、入院时尿酸水平等资料, 并随访评估2周、6个月的临床神经功能。结果入院时正常尿酸组平均尿酸为(284.5±83.6)μmol/L, 高尿酸组平均尿酸水平(455.3±78.1)μmol/L。入院时美国国立卫生研究院脑卒中量表(NIHSS)评分正常尿酸组为(11.0±6.0)分, 高尿酸组为(11.0±5.3)分, 差异无统计学意义(P=0.41)。病程2周时正常尿酸组NIHSS评分为(9.0±4.0)分, 高尿酸组为(8.0±4.0)分(P=0.038);且高尿酸组早期神经功能好转率(66例, 64.7%)明显高于正常尿酸组(152例, 53.1%)(P=0.048)。病程6个月时, 高尿酸组神经功能预后不良(mRS 3-6)发生率(52例, 51.0%)低于正常尿酸组(152例, 53.1%)(P=0.045), 差异有统计学意义。结论血清尿酸水平升高有助于缺血性脑卒中早期及远期神经功能恢复, 提示高尿酸水平对急性脑梗死具有一定的保护作用。
