高血压合并阻塞性睡眠呼吸暂停低通气综合征与胰岛素相关性研究

摘要
目的探讨高血压病合并阻塞性睡眠呼吸暂停低通气综合征(OSAHS)与胰岛素水平的相互关系。方法选择521例高血压患者,根据OSAHS程度及低通气指数(A HI)分成4组:对照组(Ⅰ组)89例,轻度OSAHS组(Ⅱ组)194例,中度OSAHS组(Ⅲ组)118例,重度OSAHS组(Ⅳ组)120例。结果重度OSAHS患者的BMI[(30.4±3.8)kg/m~2]、AHI(3.8±0.1)、空腹胰岛素[FIns,(3.08±0.26)mU/L]和胰岛素抵抗指数(2.43±0.27)高于对照组、轻、中度OSAHS组。重度OSAHS患者的最低血氧饱和度(68.7±10.8)%低于其他3组。多元回归分析表明空腹胰岛素、胰岛素抵抗指数与A HI呈正相关,与最低血氧饱和度呈负相关。结论 FIns、胰岛素抵抗指数与AHI和最低血氧饱和度密切相关,重度OSAHS高血压病患者的胰岛素抵抗表现的较为突出。
