围术期阿替普酶治疗急性脑梗塞疗效观察

摘要：

目的探讨围术期应用阿替普酶进行急性脑梗塞溶栓治疗的有效性与安全性。方法回顾性分析解放军202医院2011年1月至2017年1月在围术期应用阿替普酶进行溶栓治疗的7例急性脑梗塞患者的临床资料。结果 7例患者分别在行颈深部淋巴结活检术、前列腺电切术、经皮胆囊穿刺置管引流术、右眼玻璃体切除术、白内障手术、左股骨粗隆间骨折闭合复位PFNA内固定术、股骨头坏死人工全髋关节置换术后发生急性脑梗塞,均于围术期应用阿替普酶进行溶栓治疗。治疗后,7例患者均无颅内出血转化。其中,4例患者在溶栓24 h后神经功能恢复,2例患者在溶栓后3个月内未发生严重残障,即总有效率为85.7%(6/7);1例患者术后因继发癫痫与肺炎死亡,病死率为14.3%(1/7)。结论围术期应用阿替普酶进行急性脑梗塞溶栓治疗安全有效,可防止神经功能进一步恶化,改善患者预后。
