星状神经节阻滞对脑源性神经营养因子和白介素-8的影响

摘要
目的 观察星状神经节阻滞(SGB)对福尔马林诱导的兔伤害性反应以及背根神经节脑源性神经营养因子(BDNF)表达和血清白介素8(IL 8)浓度的影响。方法 选择日本大耳白兔,于星状神经节附近置入导管,8d后取手术成功的24只兔随机分为假手术对照组(A组)、SGB组(B组)和疼痛模型组(C组)。B组和C组在右前肢足底皮下注射3%福尔马林0. 5ml致痛,A组于相同部位注射等量的生理盐水。致痛前10min,B组经导管注射0. 25%布比卡因0. 5ml,A组和C组注射等量的生理盐水。致痛后观察各组兔的伤害性反应,并进行伤害性反应评分。致痛前10min(T0 )、致痛后10min(T1 )、60min(T2 )和120min(T3 )取兔静脉血,用放射免疫法测定血清IL 8的浓度。致痛后120min,取兔右侧C7、C8 和T1 背根神经节,用免疫组织化学方法检测BDNF的表达。结果 SGB后福尔马林诱导的Ⅱ期伤害性反应明显缓解。B组背根神经节恒定面积内BDNF阳性神经元数明显低于C组(P<0. 01),而高于A组(P<0. 01)。B组和C组T2 和T3 时间点血清IL 8浓度较T0 和T1 时间点显著升高(P<0. 01);B组T2 和T3 时间点血清IL 8浓度明显低于C组(P<0. 01)。结论 SGB可部分抑制福尔马林诱导的兔背根神经节BDNF表达和血清IL 8升高,这可能与其缓解炎症痛的机制有关。
