急性脑梗死患者血清血管内皮生长因子和S100-β蛋白水平的动态变化

摘要
目的探讨血管内皮生长因子(vascular endothelial growth factor,VEGF)和S100-β蛋白在急性脑梗死发病中的作用及与脑梗死面积、神经功能缺损评分的关系。方法应用酶联免疫吸附测定(ELISA)方法连续测定急性脑梗死患者(n=60)发病后3、7和14天VEGF和S100-β蛋白血清浓度,头颅CT或MRI测量梗死灶的最大直径,依据全国第四届脑血管病学术会议通过的脑卒中患者临床神经功能缺损程度评分标准评定神经功能缺损。结果①VEGF血清含量在脑梗死发病3天、7天和14天分别为(440±54)ng/L、(598±83)ng/L和(506±68)ng/L,均高于对照组(245±29)ng/L(P<0.01),VEGF高峰时间在发病7天(598±83)ng/L,显著高于3天和14天(P<0.05),并持续增高到发病后14天;S100-β蛋白发病3天血清浓度(0.76±0.15)μg/L高于对照组水平(0.10±0.03)μg/L(P<0.01),并在发病后7天、14天恢复正常。②VEGF的血清浓度与梗死灶的大小、神经功能缺损程度有关;S100-β蛋白与大面积梗死灶及重型脑梗死有关。结论 VEGF和S100-β蛋白血清含量在急性脑梗死患者中明显增高,表明VEGF和S100-β蛋白在发病早期参与了脑梗死病理生理过程,并可能参与了修复过程。
