急性大面积脑梗死患者血小板聚集率的动态研究

摘要
目的:探讨急性大面积脑梗死患者血小板聚集率的动态变化和相关危险因素分析。方法:选取2007年10月至2009年6月期间入住温州医学院附属第三人民医院的急性大面积脑梗死患者38例(大面积脑梗死组)、非大面积脑梗死患者85例(非大面积脑梗死组)以及非脑卒中患者40例(对照组),分别以二磷酸腺苷(ADP)和花生四烯酸(AA)作为诱导剂,采用比浊法测定血小板的最大聚集率。结果:大面积脑梗死组患者入院次日及住院1、2、3周时血小板聚集率差异无显著性(P>0.05),但入院次日血小板聚集率较非大面积脑梗死组及对照组有明显增高,且差异有显著性(P<0.05);同时大面积脑梗死组与非大面积脑梗死组相比,前者具有更多的高血压、糖尿病、高血脂及高龄患者,且在女性中发生率高(P<0.05)。结论:大面积脑梗死患者血小板聚集率明显增高,且持续3周以上。体内血小板处于过度活化状态可能在脑梗死发病中发挥重要作用。
