血压变异对急性脑梗死静脉溶栓患者出血转化的影响

摘要
目的回顾性地分析急性脑梗死患者早期静脉尿激酶溶栓24 h内血压变异对出血转化的影响。方法 2007年6月至2012年12月共有62例急性脑梗死患者应用尿激酶早期静脉溶栓患者入选,溶栓后24 h内监测BP,并于24 h、72 h、7 d分别复查头颅CT,按是否发生脑出血,分为no-HT组(54例,87.1%)和HT组(8例,12.9%)。组间比较采用t或χ2检验,将与溶栓后引起HT相关的参数(P<0.05)进行多因素Logistic回归分析。结果多因素Logistic回归分析显示,房颤(r=1.35,P<0.01)、溶栓前NIHSS评分(r=1.1,P<0.01)及SBPsv(r=1.95,P<0.001)、SBPsvmax(r=2.23,P<0.001)、DBPsv(r=2.44,P<0.001)及DBPsvmax(r=3.30,P<0.001)与溶栓后出血转化密切相关。结论血压变异与急性脑梗死静脉溶栓出血转化的发生密切相关,维持有效灌注压的关键因素之一是维持稳定的血压。
