短暂性脑缺血发作持续时间与脑缺血预适应及后继脑梗死的相关性研究

摘要
目的探讨短暂性脑缺血发作(TIA)持续时间与缺血预适应及后继脑梗死的相关性。方法选择首次急性脑梗死患者591例,按患者脑梗死前是否发生过梗死相关性TIA分为无TIA组406例和有TIA组185例,有TIA组根据TIA持续时间分为≤10 min组(35例)、>10~20 min组(86例)、>20~60 min组(40例)和>60 min组(24例)4个亚组。病情严重程度采用脑卒中神经功能缺损程度评分标准(CSS)进行评定,预后分析采用日常生活活动能力评分标准(ADL)进行评定,分析TIA持续时间与缺血预适应及后继脑梗死严重程度的相关性。结果 >10~20min组患者轻度神经功能缺损比率81%,Ⅰ级预后比率81%,与无TIA组比较有显著性差异(P均<0.01)。≤10 min组、>20~60 min组、>60 min组和无TIA组患者中重度神经缺损程度比率较高;Ⅰ级预后比较≤10 min为60%,>20~60min组62%,>60 min组58%,组间比较均无显著性差异(P均>0.05)。结论 TIA发作持续时间与缺血预适应及后继脑梗死之间有一定的相关性。
