短暂性脑缺血发作发病年限与脑梗死发生率的关系

摘要
目的探讨短暂性脑缺血发作进展为脑梗死的短期和长期相对危险度,为脑梗死的预防提供理论依据。方法采用回顾性病例对照研究,1040例脑梗死患者为脑梗死组,2982例体检人员为对照组。根据临床病例资料确定两组患者发生短暂性脑缺血发作的情况,并且采用条件性Logistic分析进行统计学分析。结果脑梗死组发生短暂性脑缺血发作187例,发生率17.98%,对照组发生短暂性脑缺血发作104例,发生率为3.49%。调整相关因素后,短暂性脑缺血发作发生后1个月内发生脑梗死的相对危险度为31.5(95%CI 11.6~88.4),1~3个月发生脑梗死的相对危险度为19.2(95%CI 8.6~41.7),4~6个月发生脑梗死的相对危险度为3.98(95%CI 1.38~7.93),5年后发生脑梗死的相对危险度为2.14(95%CI 1.22~2.85)。结论短暂性脑缺血发作发生后3个月内发生脑梗死的相对危险程度最高,是预防的关键时期。
