高同型半胱氨酸血症危害及致病机制研究进展

摘要
同型半胱氨酸(homocysteine,Hcy)是多种疾病的独立危险因素,与心脑血管疾病、神经系统疾病、糖尿病及其并发症等密切相关。高Hcy可以明显增加脑卒中患者心脑血管事件再发风险和脑卒中复发风险,升高全因死亡率;Hcy超过14μmol·L-1,老年痴呆的危险性增加1.8倍;糖尿病患者血浆Hcy水平每升高5μmol·L-1,未来5年内死亡率增加5倍。高Hcy可通过氧化应激、损伤血管内皮细胞、刺激血管平滑肌细胞增殖、影响凝血、脂质代谢功能,改变基因组甲基化水平等多种机制诱导疾病的发生。
