系统评价再评价纳入研究的质量评价方法调查

摘要：

目的对当前发表的系统评价再评价(overviews of reviews,简称Overviews)纳入研究的质量评价方法进行调查分析,为Overviews制作者提供方法学参考。方法计算机检索Cochrane Library(2010年第1期)、PubMed、EMbase和中国生物医学文献数据库(CBM),检索时间截至2009年12月,遴选出具代表性的Overviews。然后提取其质量评价标准等相关资料进行分析,并对主要质量评价标准进行讨论分析。结果共遴选出43个具有代表性的Overviews,其中32个(74.4%)对纳入的系统评价进行了方法学质量评估,15个采用OQAQ标准(34.9%)、3个采用AMSTAR标准(7.0%)、2个采用DARE清单(4.6%)、2个采用Assendel标准(4.6%)、1个采用E ective Public Health Practice Project标准(2.3%)、6个采用自定义标准(14.0%),1个采用综合标准(2.3%),2个为其他(4.6%)。有10个研究(23.6%)进行了证据质量评价,其中8个研究(18.6%)应用GRADE分级系统进行评价,7个研究(16.3%)同时应用了证据质量评价和GRADE分级评价方法。结论现有Overviews对纳入研究的质量评价包括方法学质量和证据质量,但大多数现已发表的Overviews文献的质量评价实施并不完整。同时,现已发表Overviews中采用的方法学质量评价标准众多,尚无统一标准。鉴于OQAQ标准和AMSTAR标准评价内容全面,实施简易,其应用相对广泛,并得到了推荐,建议Overviews作者合理选择应用。GRADE系统相比其他证据质量分级系统更具系统性和全面性,推荐制作者在研究过程中正确使用,使Overviews研究结果更加完整,便于临床推广应用。
